FreeStyle Libre 2 sensors are being discontinued at the end of August 2025.

All existing patients using the FreeStyle Libre 2 sensor will require their repeat prescription to be changed to the FreeStyle Libre 2 Plus sensor. Inform patients that their prescription will be changed and advise them to use up existing supplies of FreeStyle Libre 2 sensors first. No app updates or change of reader are required.
Only FreeStyle Libre 2 Plus should be used for new initiations.

FreeStyle Libre 2 Plus sensors last 15 days, opposed to FreeStyle Libre 2 sensors which last 14 days.

FreeStyle Libre 2 Plus prescription quantities: 2 sensors = 30 days, 4 sensors = 60 days. Maximum 24 sensors to be prescribed per year.

 

NHS Kent and Medway ICB have agreed that prescribing of continuous glucose meters (CGM) can be for those people living with diabetes in the eligible cohorts defined by NICE criteria NG17, NG18 and NG 28. This is for those devices which can be prescribed on FP10 prescription only.

Specialist initiation: These are healthcare professionals in secondary care, community, or primary care healthcare professionals who have undertaken appropriate training for the initiation and monitoring use of these devices (see policy and document attached for further information on this) and can make a shared decision with patients

Dexcom ONE is due to be discontinued in the near future- patients should be moved to Dexcom One plus at their next appointment using up any existing supplies.

Specialist initiation: These are healthcare professionals in secondary care, community, or primary care healthcare professionals who have undertaken appropriate training for the initiation and monitoring use of these devices (see policy and document attached for further information on this) and can make a shared decision with patients

Only for those patients who are currently prescribed Freestyle Libre 1 or 2. See NICE guidelines for more information

 

Only one transmitter once every 3 months – no more than 4 transmitters per year.

This item has an inbuilt transmitter so no other items are required to be prescribed with this item. One sensor lasts 10 days

Please note each sensor lasts 15 days. Can be used from age 2 upwards. Can be used in hybrid closed loop technology where initiated by specialist centre.
