Dartford, Gravesham and Swanley Joint Formulary

Welcome to the Dartford, Gravesham and Swanley joint formulary which contains the list of medicines approved for prescribing within the Dartford, Gravesham and Swanley local economy. 

The medicines, recommendations and guidelines have been approved by the Prescribing and Formulary Group (PFG) consisting of representatives from the Dartford and Gravesham NHS Trust and the Dartford, Gravesham and Swanley Clinical Commissioning Group (CCG).

The medicines have been selected on the basis of clinical effectiveness, cost effectiveness and patient acceptability.

All medicines with positive NICE Technology Appraisal recommendations, that are relevant to the services provided, are automatically incorporated into the formulary in accordance to the mandate in the Innovation Health and Wealth document, December 2011.
